Patient Safety: Past, present and future

Patient Safety is not a new idea, but the drive to embed it into every decision and process within the NHS is. Recognising the distance we have come so far on both nationally and locally is essential in achieving future success.

This session will provide not only an overview of the Patient Safety Campaign to date, but an essential review of what to expect in the future. What success has the campaign achieved to date? What winning strategies can be shared and embedded across the NHS? And why is the recession an opportunity not a threat to patient safety?
Discover:

  • Examples of highly successful projects
  • An overview of future agendas to be aware of

A masterclass in language, policy and patient safety resources

The vast increase in the emphasis and noise on patient safety has resulted in a step increase in the amount of policy, processes and technical lingo. Many systems are in place to support the NHS in its quest to patient safety, but knowing where to look, who to contact and what is best practice can be challenging.

This session will provide an overview of the policy that effect patient safety today, an introduction to the related acronyms and directions to the resources you should be using everyday.
Discover:

  • Clarification on key policies and information
  • A recommended reading list and contact information

How to become a patient safety leader

Patient Safety will never become a reality in the UK without exceptional leadership. But this leadership must take two forms: with guidance from above from committed national leads and dedicated boards and with frontline leaders delivering engaged practitioners. It is only with a combination of this top down guidance and the development of a grass roots social movement that change will be embedded and safer care delivered.

This session will provide an overview of the new national leadership bodies and their role in patient safety training. Plus clarify how organisations can develop more competent leadership programmes with patient safety at their heart in an environment where budgets and resources are being increasingly stretched.
Discover:

  • The skills you need to develop to be a patient safety leader
  • Learnings from national programmes including LIPS and Patient Safety First

Developing safer cultures

The NHS is the third largest employer in the world, and in its 60 year history has become increasingly complex and diverse. Actually achieving change within these ingrained cultures presents an overwhelming challenge. But it is a challenge that some people have cracked – organisations across the UK have successfully changed their cultures and delivered improvements for the safety of their patients.

This session will help you to understand the key levers available to you to drive change in practice. Whatever your role, embrace the tools available to deliver change and develop a culture within your organisation that will support patient safety.
Discover:

  • The techniques available to empower you to change practice
  • Influences on team working and how to improve care on the ground

Safety improvement – which tools and techniques could work for you?

There is now a wide range of tools, techniques and methodologies available for patient safety improvement projects.  However, many are relatively new and there is considerable variation in the extent they have been tried and tested.   This is an opportunity to hear from a leading figure in patient safety improvement who can provide an invaluable route-map through the improvement maze.  Whichever health sector or organisation you work within, some tools and techniques will be more suited to your needs than others.  As well as providing an overview of the tools and techniques available, during this session you will learn about:

  • Programmes developed by the NHS Institute for Innovation and Improvement’s Safer Care team including the highly acclaimed leading Improvement in Patient Safety (LIPS) programmes for acute and community, mental health and primary care
  • The role of executives and leaders in patient safety improvement
  • How an understanding of case note review with a trigger tool, reliability science, human factors and measurement will help you improve the care you provide

Improving risk management, assurance and delivering effective governance

Organisations must have reliable, robust systems in place to assure the quality of care they provide. High profile examples of multisystem failures such as Mid Staffordshire have shown that failings in the provision of care can go unrecognised and unmitigated. New challenges are arising in ensuring the successful integration of NPSA reports and CQC requirements from Board to ward.

This session will clarify the latest requirements in risk management, assurance and governance and demonstrate techniques to ensure uptake. It will review why governance sometime fails and explore how this can be prevented.
Discover:

  • How an organisation must assure itself around safety
  • What governance should be in place

Infection Prevention

HCAIs are one of the most high profile issues in the NHS. The majority are preventable and it is the responsibility of all healthcare professionals to minimise transfer and spread of infections. Recent marketing campaigns are having an impact on infection rates but embedding best practice and implementing new technologies means there is still an opportunity to improve.

The session will explore how high performing Trusts have driven a dramatic reduction in infection. It is an opportunity for everyone to share best practice and understand how this can be built on, irrespective of the type of organisation they operate in.
Discover:

  • How your organisation can achieve a zero case rate
  • Strategies to engage your stakeholders in reducing HCAIs
  • Techniques and technology that will assist you

Successfully managing medicines

Managing the delivery of drugs within the NHS is one of the key areas that patient safety must focus on. In primary, secondary and tertiary care managing this area is exceptionally challenging. It is essential organisations and individuals learn to be proactive around risks that exist and as new processes, systems and information technology are implemented they seize the opportunity to design out error.

This session will showcase the innovations occurring in this area. It will review the challenges around medicine management across different healthcare settings and explore the role of pharmacists in patient safety as a whole.
Discover:

  • How to work better in partnership with pharmaceutical services and vice versa
  • Techniques to engage staff in medicines management
